Jérémy Geeraert ist derzeit Postdoktorand am Cesdip im Centre national de la recherche scientifique (CNRS) und ist Teil des europäischen Projekts "CrimScapes : Navigating citizenship through European landscapes of criminalisation" (Norface). Er führt ein Forschungsprojekt über die Kriminalisierung der Migrationshilfe durch, das Navigationspraktiken und die Konstituierung von Verbrechensfiguren in den Bereichen der Suche und Rettung im Mittelmeer und des Aktivismus an der französisch-italienischen Grenze untersucht. Zwischen 2018 und 2021 war er sukzessive Stipendiat der Stiftung des Abgeordnetenhauses von Berlin, der Fritz Thyssen Stiftung und der Alexander Humboldt Stiftung und führte ein Forschungsprojekt zu den Zusammenhängen zwischen Migration und Gesundheitspolitik durch. Sein Forschungsschwerpunkt liegt auf dem Zugang zur Gesundheitsversorgung für Migranten in Situationen mehrfacher Gefährdung und ohne Krankenversicherungsschutz in Deutschland. Seine im Oktober 2017 verteidigte Doktorarbeit befasste sich mit dem Zugang zur Gesundheitsversorgung für Migranten ohne Krankenversicherungsschutz in öffentlichen Krankenhäusern in Frankreich. Das Buch, das auf dieser Arbeit basiert, wird derzeit veröffentlicht.;

Jérémy Geeraert est sociologue, spécialisé en santé et en migration. Il a fait ses études de sociologie et de sciences sociales à l'université Paris René Descartes et à l'EHESS/Paris 13. Il a soutenu sa thèse en sociologie en octobre 2017 à l'université Paris 13 sur le sujet de l'accès aux soins des personnes éloignées du système de santé en prenant l'exemple des Permanences d'accès aux soins de santé à l'hôpital public. Il a été chargé d'enseignement à l'université Paris 13 et à la Humboldt dans le département d'ethnologie européenne. Il a été dotorant associé au Centre Marc Bloch entre 2014 et 2017. Entre 2018 et 2021, Jérémy a poursuivi ses recherches dans le cadre d'un postdoctorat au Centre Marc Bloch et à l'institut d'ethnologie européenne de l'université Humboldt au cours duquel il s'est intéressé dans une démarche comparative à l'accès aux soins des étrangers au statut de séjour précaire en France et en Allemagne.  Depuis 2021, il mène une recherche dans le cadre du projet de recherche européen "CrimScapes : Navigating citizenship through European landscapes of criminalisation" sur la criminalisation des activités de sauvetage en mer dans la méditerranée et d'aide à la migration à la frontière franco-italienne.;

Jérémy Geeraert is currently a postdoctoral researcher at Cesdip at Centre national de la recherche scientifique (CNRS) and is part of the European project 'CrimScapes : Navigating citizenship through European landscapes of criminalisation' (Norface). He is carrying out a research project on the criminalisation of migration help which explores navigation practices and the constitution of figure of crime in the fields of search and rescue in the Mediterranean and of activism at the French-Italian border. Between 2018 and 2021, he was successively grantholder of the Foundation of the Berlin House of Representatives, of the Fritz Thyssen Foundation and of the Alexander Humboldt Foundation and carried out a research project on the interrelationships between migration and health policies. His research focuses on the access to health care for migrants in situations of multiple vulnerabilities and without health coverage in Germany. His doctoral thesis, defended in October 2017, focused on access to healthcare for migrants without health coverage in public hospitals in France. The book based on this thesis is currently being published.

Zusammenfassung

This thesis explores how the “social question” (question sociale) is managed as an issue of health at public hospitals through the lens of power relations and health care structures. To this end, the sociological study examines the “healthcare access unit” (Permanence d’accès aux soins de santé, PASS) of public hospitals in France, which are reserved for persons who are excluded from the healthcare system – most notably irregular migrants. The employed methods combine a socio-historical analysis with ethnographic fieldwork — including participant observation and semi-structured interviews (n=40) — conducted over the course of three years in sixteen PASS units based at eight different hospitals, and in the broader structures within which they exist (i.e. associations and institutional guardianships). In the first section, a genealogy of the “social question” in health in France demonstrates how space for the healthcare of poverty has emerged at the end of the 20th century, which specifically targets social groups among the most destitute. This space of a particular type of care was increasingly institutionalized in the public hospital system during the 1990’s, embedding itself in both the field of public health and in the fight against social exclusion. This space fulfills several (bio)political objectives: fighting social exclusion, ensuring a right to health, and protecting the health of the population as a whole. Second, an analysis of PASS units in the organisation of public hospitals exposes how they are weakened through two dominant and competing models (technical specialization and new public management). Faced with this situation, varied strategies (ex. of adaptation and empowerment) are employed by different actors inside and outside of the hospital. Lastly, a third section lays out the categories of patients that are produced during interactions with professionals from PASS units. It exposes the role of these categories in the stratification of the health care system along its lowest margins. Patients are divided into more or less legitimate groups - based on criteria of citizenship and of solvency – each of which are afforded differentiated values of life. These dynamics are characterized by a permanent tension between inclusion and exclusion in the field, and by a differentiated distribution of health care. In such a context, an individualized and flexible government allows for the pursuit of these multiple (bio)political objectives. Keywords: healthcare access;

Ce travail porte sur l’étude du traitement de la question sociale dans le domaine de la santé à l’hôpital public au prisme des rapports de pouvoir et de l’organisation du système de santé. Ainsi, l’enquête sociologique a porté sur les Permanences d’accès aux soins de santé (PASS) présentes au sein d’hôpitaux publics en France et dont l’accès est réservé aux personnes éloignées du système de santé, dont le cas emblématique est celui de l’étranger sans papier. La méthode a consisté à combiner des éléments sociohistoriques et une enquête empirique qui s’est déroulée sur trois années auprès de seize PASS implantées dans huit villes et au sein d’organismes en lien avec elles, telles des associations ou encore les tutelles institutionnelles. Des observations et des entretiens semi-directifs auprès de professionnels ont été réalisés (n=40). D’abord, l’étude de la généalogie de la question sociale en santé en France montre comment s’est progressivement constitué à la fin du xxe siècle un espace du soin de la précarité visant à prendre spécifiquement en charge des groupes sociaux parmi les plus défavorisés. Cet espace de soin spécifique a connu une institutionnalisation dans le système hospitalier public français au cours des années 1990, s’inscrivant à la fois dans le champ de la santé publique et dans celui de la lutte contre l’exclusion. Il remplit des objectifs (bio-)politiques multiples : lutter contre l’exclusion sociale, garantir un droit à la santé, protéger la santé de la population générale. En second, l’étude des PASS dans l’organisation hospitalière conclut à leur mise en difficulté par les deux modèles dominants concurrents (spécialisation technique et néo-managérial). Face à cette situation, diverses stratégies (d’adaptation et d’autonomisation) sont déployées par différents acteurs à l’intérieur et à l’extérieur de l’hôpital. Enfin un troisième résultat relève de l’analyse des catégories de patients produites lors des interactions avec les professionnels des PASS. Elle met en lumière le rôle qu’elles jouent dans la stratification du système de santé à ses marges les plus basses. Les patients sont ainsi séparés en groupes plus ou moins légitimes selon des critères de citoyenneté et de solvabilité auxquels sont attribuées des valeurs de la vie différenciées. Cela se caractérise dans le quotidien des PASS, par une tension permanente entre inclusion et exclusion du patient et une distribution différenciée des soins. Un gouvernement individualisé et flexible permet de poursuivre dans un tel contexte la multiplicité des objectifs (bio-)politiques.
